Job Summary:We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, assisting with daily living activities, administering medications, and coordinating care with our multidisciplinary team.
Responsibilities:- Implementing physicians' prescribed plans of care
- Conducting patient assessments and coordinating care
- Administering prescribed medications, treatments, and therapies
- Documenting care provided in our electronic system
- Providing health education and training to patients and their families
- Active, unencumbered license (RN/LPN/LVN) in the state of practice
- Current CPR certification
- TB skin test (current within last 12 months)
- At least six months of prior hands-on nursing experience
- Reliable transportation
