Aveanna Healthcare is a leading provider of home health services to medically fragile children and adults. We are seeking a skilled Private Duty Nurse LVN to join our team.

Job Summary:

  • Provide one-on-one care to patients in their homes, assisting with daily living activities and medical needs.
  • Implement physicians' prescribed plans of care, including administering medication and treatments.
  • Conduct patient assessments and coordinate care with healthcare teams.
  • Document care provided in a timely and compliant manner.

Responsibilities:

  • Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Administer medication and treatments as prescribed by physicians.
  • Conduct patient assessments to identify changing needs and develop care plans.
  • Coordinate care with healthcare teams, including physicians, therapists, and social workers.
  • Document care provided in a timely and compliant manner.

Requirements:

  • Active, unencumbered license (RN/LPN/LVN) in the state of practice.
  • Current CPR certification from American Red Cross or American Heart Association.
  • TB skin test (current within last 12 months).
  • Six months prior hands-on nursing experience preferred.
