About the Role

As a Private Duty Nurse LVN, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our care team to ensure seamless care coordination and exceptional patient outcomes.

Responsibilities
  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease management
  • Communicate effectively with patients, families, and healthcare teams

Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
