Job Description

As a key member of the Heritage Gardens Health Care Center team, the Clinical Liaison/Admissions Coordinator plays a vital role in fostering relationships between patients and our facility. This role involves establishing patients' eligibility for care, communicating with families, and interacting with a wide range of staff members.

Key Responsibilities:

  • Establish patients' eligibility for admission to the facility.
  • Review patients' medical records.
  • Explain care options to patients and their families.
  • Interact with a variety of hospital staff.
  • Coordinate with the facility staff to ensure a smooth transition for patients.
  • Schedule and attend community events.
  • Develop relationships in the community on behalf of the facility.
  • Work with the marketing team to promote the facility's strengths and services.
  • Train new employees who serve as Clinical Liaisons.
  • Build relationships with managed care companies on behalf of PHC.

Administrative Responsibilities:

  • Meet with the Quality Assurance team quarterly.
  • Make facility rounds weekly to survey patients and ensure their needs are met.
  • Evaluate customer service quarterly and provide in-services as needed.
  • Work closely with the Contracts Manager to ensure high-quality rates in the area.
  • Attend weekly Rehab Meetings and provide updates on patients to HMO.

Consumer Service/Guest Relations:

  • Present a professional image to consumers through dress, behavior, and speech.
  • Adhere to Company standards for resolving consumer concerns.
  • Adhere to all consumer and resident rights.
  • Ensure residents and families feel welcome and comfortable.
  • Demonstrate exceptional verbal and written communication skills.
  • Experience in general office procedures, including typing and computer skills.
  • Must be customer service oriented.
  • Experience working in the healthcare industry.

Requirements:

  • Practical experience in a clinical setting.
  • Ability to work after hours and weekends when needed.
  • Licensed Vocational Nurse Credentials Preferred.
  • CPR Certification Preferred.
