Clinical Practice Leader

LifeSpan is seeking a highly skilled and experienced Clinical Practice Leader to join our team. As a key member of our nursing leadership team, you will provide expert nursing guidance and support to our inpatient units, focusing on high-risk and decompensating patients on the med/surg floors. Your primary responsibility will be to facilitate timely transfers of patients to a higher level of care and assist the Nursing Supervisor with appropriate utilization of resources.
Key Responsibilities- Recognize and report deviations from patient or established norms, identifying and assessing emergency situations, and initiating appropriate responses and interventions.
- Define nursing diagnoses and expected patient outcomes, modifying the nursing plan of care as needed, and documenting on appropriate nursing assessment forms.
- Integrate complete physical assessments with nursing assessments and record reviews in developing a comprehensive and individualized plan of care.
- Utilize established protocols and collaborate with the healthcare team to facilitate therapeutic interventions and attainment of desired patient outcomes.
- Maintain knowledge of contemporary professional nursing practice within the legal framework of the Rhode Island Nurse Practice Act, attending training and retraining as needed to maintain current knowledge of hospital and patient care practices and procedures.
- Participate in hospital departmental and system-wide committees and teams as deemed necessary, demonstrating self-direction in maintaining and advancing clinical and professional competency.
- Collaborate with the Nursing Supervisor to provide support to nursing units, proactively rounding on high-risk patients, and assisting in the facilitation of transfers to higher levels of care.
- Plan and manage patient flow to provide optimal patient care services through the institution, working collaboratively with all departments and Lifespan affiliates.
- Graduate of a school of nursing with a Baccalaureate in Nursing, current license to practice as a Registered Nurse in the State of Rhode Island, and conformity to the Code of Professional Nurses.
- Understanding of the provisions of the Nurse Practice Act of the state of Rhode Island.
- Demonstrated clinical leadership as a Charge Nurse, Preceptor, and/or shared governance council lead.
- Five years of progressively responsible clinical experience in medical/surgical nursing, with two years of critical care or emergency department experience preferred.
Prolonged standing and walking, exposed to all patient elements, may be required to lift/turn patients and equipment, and repeated bending, squatting, and stooping.
