CarePartners is seeking a highly skilled Home Care Clinical Coordinator to join our team. As a key member of our PACE program, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to our participants.
Responsibilities- Coordinate the day-to-day operations of in-home services for our PACE participants and families.
- Act as a liaison between the contracted home health agency and the PACE program, coordinating homecare services and ensuring seamless communication.
- Utilize nursing skills to assess participants in their home environment, coordinate plans of care, and provide treatments and health education as needed.
- Participate in interdisciplinary team meetings to ensure compliance with competency requirements, maintain accurate medical records, and provide thorough documentation.
- Develop and implement health and wellness plans in collaboration with participants, families, and healthcare providers.
- Current licensure as a registered nurse with the North Carolina Board of Nursing.
- Minimum of two years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting.
- One year of experience working with a frail or elderly population.
- Willingness to work in home environments with diverse values and standards.
- Computer literacy with EMR and familiarity with telehealth and video consultations.
- Nursing experience in a community home-based setting.
- ACTT RN or Mental Health experience.
