Home Health Clinical Coordinator

About the Role
CarePartners is seeking a highly skilled Home Health Clinical Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for coordinating in-home services for our PACE participants and their families.
Responsibilities
- Coordinate day-to-day operations of in-home services for your team of participants
- Act as liaison between contracted home health agency and PACE program, coordinating homecare Nurses, Home Health Aides, Homecare Services Non-Skilled Aides, and Homecare Schedulers
- Utilize nursing skills to assess participants in their home environment and coordinate plans of care with appropriate resources, providing treatments and health education as needed
- Participate on interdisciplinary team to ensure compliance with competency requirements, maintaining medical records and thorough documentation
- Plan and utilize telehealth, assistive technology, and community resources to create and implement health and wellness in homes
Requirements
- Required Education: Bachelor of Science in Nursing
- Required License: Current licensure as registered nurse with the North Carolina Board of Nursing
- Required Experience: Two years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting; minimum of one year working with a frail or elderly population
- Preferred Experience: Computer literacy with EMR and familiarity with telehealth and providing consultations over a video platform; nursing in a community home-based setting preferred
