DePaul Community Resources is seeking a dedicated Case Management Assistant to join our team. This position is primarily remote, with occasional requirements to attend meetings at the designated office.


POSITION SUMMARY:

The Case Management Assistant plays a crucial role in the ID/DD Services Division by ensuring that documentation, case notes, and various records are meticulously reviewed, updated, and completed. This role supports the Residential Coordinators by managing specific case management documentation, as directed by the Residential Manager, based on the needs of the caseloads.


SUPERVISION:

This position reports directly to the Regional Manager.


KEY RESPONSIBILITIES:

The following responsibilities are essential to the role:

  • Review and assess documentation such as monthly progress notes and home visit assessments, providing constructive feedback to the assigned Residential Coordinator and Supervisor.
  • Conduct thorough reviews of assigned documentation to facilitate quarterly person-centered reviews for submission.
  • Draft initial versions of annual plans by incorporating information from assessments and evaluations, forwarding these drafts for completion.
  • Adhere to deadlines set by the electronic health record system and supervisory guidelines, ensuring compliance with relevant policies and regulations.
  • Complete required training as assigned.
  • Identify and report trends or concerns in case records to the assigned Residential Coordinator and Supervisor.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

The following attributes are necessary for success in this role:

  • Familiarity with records administration techniques and procedures.
  • Proficiency in Microsoft Office Suite and virtual meeting applications.
  • Understanding of DePaul's policies and the regulations set by governing agencies.
  • Ability to communicate effectively and maintain professionalism in all interactions.
  • Strong organizational skills to manage multiple priorities and meet deadlines.

MINIMUM QUALIFICATIONS:

An Associate's degree in a human services field and one year of experience with the ID/DD population, or two years of equivalent experience, is required. A combination of education and experience may be considered.


WORK ENVIRONMENT:

This role is primarily office-based with occasional fieldwork. Work hours may vary, and the environment may be subject to interruptions.


PHYSICAL DEMANDS:

Physical activities include dexterity for operating office equipment, effective communication, and the ability to navigate various locations as needed.


ADDITIONAL REQUIREMENTS:

Post-offer requirements may include a driving record check, drug screening, and background checks.
