The incumbent performs complex tasks to support individuals in achieving their goals and improving their overall well-being.
Key Responsibilities- Develop and implement individualized case plans to address the unique needs of clients
- Collaborate with healthcare providers, social services, and other stakeholders to ensure comprehensive care
- Provide education and support to clients and their families on mental health and substance use disorder topics
- Monitor client progress and make adjustments to case plans as needed
- Document all interactions and case management activities in a timely and accurate manner
The ideal candidate will possess a strong understanding of behavioral health principles and practices, as well as excellent communication and interpersonal skills. A bachelor's degree in a related field and relevant work experience are required.
