Hospice RN Case Manager Position

As a Hospice RN Case Manager at Interim HealthCare Staffing, you will play a vital role in providing exceptional care to patients and their families. Your expertise and compassion will make a significant difference in the lives of those we serve.
Key Responsibilities:
- Maintain a specified case management load, ensuring timely and effective care delivery.
- Conduct comprehensive assessments of patients and their families to determine hospice needs, including a thorough review of medical history and physical assessment.
- Participate in on-call duties as defined by the office on-call policy, providing 24/7 support to patients and families.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
- Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services and achievement of hospice care goals.
- Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.
- Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
- Facilitate consideration of alternative levels of care when indicated by patient or family status/needs.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina, with an active and good standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
- If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
