Hospice RN Case Manager

As a Hospice RN Case Manager with Interim HealthCare Staffing, you will play a vital role in providing compassionate and high-quality care to patients and their families. Your expertise and dedication will make a significant difference in the lives of those we serve.
Key Responsibilities:- Conduct comprehensive assessments of patients' physical and psychosocial status to determine their needs and develop individualized care plans.
- Collaborate with the Interdisciplinary Group (IDG) to ensure the delivery of coordinated and effective care.
- Coordinate the provision of services, including hospice care, respite care, and other support services, to meet the needs of patients and their families.
- Supervise and direct the work of LPNs, Hospice Aides, and Homemakers to ensure the delivery of high-quality care.
- Facilitate the consideration of alternative levels of care, such as inpatient or continuous care, when necessary.
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge.
- One (1) year of hospice care experience.
