Hospice RN Case Manager Position

Interim HealthCare Staffing is seeking a skilled Registered Nurse to fill the role of Hospice RN Case Manager. This position requires a compassionate and detail-oriented individual to provide exceptional care to patients and their families.
Key Responsibilities:
- Maintain a specified case management load to ensure timely and effective care delivery.
- Conduct comprehensive assessments of patients and their families to determine hospice needs, including a history of current and previous illnesses, and a physical assessment of the patient.
- Participate in on-call duties as defined by the office on-call policy to ensure 24/7 coverage.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), the patient, and the attending physician.
- Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services and achievement of hospice care goals.
- Evaluate and coordinate the need for services provided to patients and their families in accordance with the IDG plan of care.
- Supervise and direct the services provided by and LPN/LVN, Hospice Aide, and Homemaker assigned to patients and their families.
- Facilitate the consideration of another level of care when indicated by the patient's or family's status or needs.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina, with an active and good standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge as deemed appropriate by the DHCS.
- If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months, or evidence of completion of a company-approved Preceptor Program.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
