Job Purpose

To enhance the comfort of hospice patients and provide temporary respite for families by delivering essential personal care services.

Key Responsibilities

  • Uphold the principles and standards of hospice care.
  • Engage effectively with diverse age groups.
  • Work independently and collaboratively within a multidisciplinary team.
  • Communicate clearly and effectively in both verbal and written forms.
  • Maintain confidentiality in all matters.
  • Adapt to a flexible work schedule.
  • Deliver patient care in alignment with established policies and procedures, adhering to the CNA/PCA care plan.
  • Assist with personal hygiene tasks such as bathing, oral care, skin care, and hair care.
  • Support patient mobility, including transfers and ambulation.
  • Aid in meal preparation and assist with feeding as necessary.
  • Ensure a clean, safe, and comfortable environment for patients.
  • Be attuned to the emotional needs of patients and understand the psychological challenges associated with their conditions.
  • Collaborate with the primary nurse and health services director to coordinate care.
  • Keep accurate and current records of patient visits.
  • Provide comfort care related to patient positioning and skin integrity.
  • Participate in interdisciplinary team meetings to contribute to care planning and evaluation.
  • Report patient status and any concerns to supervisory staff, following protocols for emergencies.
  • Deliver dignified and respectful care to all patients.
  • Perform additional duties as assigned by supervisory personnel.

  • Demonstrate maturity and the ability to manage job demands effectively.
  • Possess reading, writing, and comprehension skills to follow instructions.
  • Exhibit empathy towards terminally ill patients and their families.
  • Show understanding and commitment to the mission of PruittHealth Hospice.
  • Exhibit strong listening skills.
  • Document services rendered in accordance with agency policies and the CNA/PCA care plan.
  • Complete necessary training to perform assigned duties effectively.
  • Engage in agency evaluations and necessary reporting.
  • Participate in ongoing educational programs to stay updated in the field and maintain required licensure/certification.
  • Attend mandatory training sessions.
  • Respect patients' rights to fair treatment, privacy, and individuality.
  • Adhere to corporate compliance standards.
  • Report any job-related hazards and follow safety regulations.
  • Comply with established safety procedures when performing tasks.
  • Carry out other related duties as directed by a supervisor.

Requirements

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

  • Active, current, unrestricted Certified Nurse Assistant (CNA) certification in the state of practice.
