Job SummaryThe Ambulatory Care Nurse (ACN) plays a critical role in providing nursing care management services to patients in a community setting. As a key member of the interdisciplinary healthcare team, the ACN works collaboratively with physicians and other healthcare professionals to identify at-risk patient populations and develop effective care plans.
Key Responsibilities- Provide nursing care management services to assigned patients, including assessment, planning, implementation, and evaluation of care.
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
- Identify at-risk patient populations and develop strategies to prevent hospitalization and improve health outcomes.
- Act as a patient/family advocate and communicate effectively with patients, families, and healthcare teams.
- Support the mission, values, and management of patient care services at WakeMed.
- 5 years of experience in a nursing-related field.
- Diploma or Associate's degree in nursing required; Bachelor's degree in nursing preferred.
- Registered Nurse licensure required.
Monday-Thursday: 7:30am-5:00pm; Friday: 8:00am-12:00pm. No weekends or call requirements.
