Job Responsibilities- Provide nursing care management services to assigned populations
- Collaborate with physicians to identify at-risk patient populations and develop care plans
- Act as a patient/family advocate and communicate effectively with the healthcare team and community resources
- Support the mission, values, and management of patient care services
- 2 years of nursing experience or equivalent education
- Diploma or Associate's degree in nursing required; Bachelor's degree preferred
- Registered Nurse licensure required
Monday through Friday, 8 am to 4:30 pm; no weekends or call requirements
