Job SummaryThe Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. This role utilizes a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.
Key Responsibilities- Identify at-risk patient populations requiring care management services in collaboration with physicians.
- Serve as a patient/family advocate and communication link between physicians, patients, families, and interdisciplinary healthcare teams, as well as community resources.
- Support the mission, values, and management of patient care services, actively incorporating ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
- No experience required.
- Diploma in Nursing or Associate's Degree in Nursing required.
- Registered Nurse licensure required.
