Job Summary

The Ambulatory Care Nurse III is a registered nurse responsible for providing nursing care management services to assigned populations. This role requires a collaborative approach to anticipate, integrate, and coordinate patient care plans to maximize the ability to remain in a community setting.

Key Responsibilities
  • Identify at-risk patient populations who require care management services in collaboration with physicians.
  • Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
  • Support the mission, values, and management of patient care services at WakeMed.
  • Actively support and incorporate the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Requirements
  • 5 years of experience in a nursing-related area.
  • Diploma in Nursing or Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
  • Registered Nurse licensure required.
  • Monday-Thursday 7:30am-5:00pm; Friday 8:00am-12:00pm schedule.
  • No weekend requirements.
  • No call requirements.
