Position InformationThe Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.
In collaboration with the physician, the nurse identifies at-risk patient populations who require care management services. The ACN serves as a patient/family advocate and acts as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
The Ambulatory Care Nurse understands the needs of the organization and supports the mission, values, and management of patient care services. The ACN actively supports and incorporates the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Experience Requirements2 years of nursing experience or no experience required with preferred education.
Education RequiredDiploma in nursing or Associate's degree in nursing required, and Bachelor's degree in nursing preferred.
Licensure/Certification RequirementsRegistered Nurse required.
Hours of Work8 am to 4:30 pm, Monday through Friday.
Weekend RequirementsNo weekends.
Call RequirementsNA.
