Job Summary

We are seeking a skilled Nurse Care Coordinator to join our Heart Failure Management team. The successful candidate will be responsible for coordinating patient care, communicating with healthcare teams, and ensuring seamless transitions between care settings.

Key Responsibilities
  • Coordinate patient care plans, ensuring timely and efficient delivery of services
  • Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care
  • Identify and address care gaps, ensuring patients receive necessary interventions and services
  • Collaborate with multidisciplinary teams to develop and implement care plans
  • Monitor patient progress, making adjustments to care plans as needed
Requirements
  • Graduate of an accredited nursing program
  • Bachelor's degree in Nursing (BSN) required
  • Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board
  • Care Management Experience preferred
  • Three (3) years of healthcare experience required
  • BLS Healthcare Provider certifications required

Compensation
