Job Summary:

Piedmont Atlanta is seeking a skilled RN Evaluation Coordinator to join our Heart Transplant team. As a key member of our multidisciplinary team, you will play a pivotal role in assisting patients through the evaluation process for heart transplant and LVAD candidacy.

Job Purpose:

Participate in the development, implementation, and maintenance of the left ventricular assist device (LVAD) program and the heart transplant program. Coordinate and manage the patient care of this high-risk patient population throughout the continuum.

Responsibilities:

  • Assist patients through the evaluation process for heart transplant and LVAD candidacy
  • Coordinate and manage patient care throughout the continuum
  • Participate in the development, implementation, and maintenance of the LVAD program and the heart transplant program
  • Collaborate with multidisciplinary team members, including cardiothoracic surgeons, pharmacists, registered nurses, social workers, and dietitians

Qualifications:

  • Graduate from a Registered Nurse Program
  • Two (2) years of professional clinical experience required
  • Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License
  • BLS and ACLS certification required
  • Cardiovascular focus experience preferred
  • One (1) year experience caring for LVAD patients and heart transplant patients preferred

Requirements:

  • Leadership qualities, including time management, verbal and written communication skills, listening skills, decision making, priority setting, and work organization skills
  • Self-directed and independent judgment
  • Effective communication skills and team building skills
  • Demonstrate compassionate patient communication
