Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at LifeBridge Health. As a LPN in our Long-Term Care unit, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare organization.

Key Responsibilities
  • Develop and implement individualized nursing care plans to meet the unique needs of our residents
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
  • Graduate of an accredited school of nursing with a current LPN license
  • Basic computer skills and proficiency in electronic medical records
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
  • Strong communication and interpersonal skills, with a focus on patient-centered care
