We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at LifeBridge Health. As a member of our healthcare team, you will have the opportunity to make a meaningful difference in the lives of our patients and residents.

Job Summary:

Under the supervision of the Registered Nurse/Director of Nursing, the Licensed Practical Nurse will utilize a general understanding of the principles of nursing and basic physical assessment skills in the development of and implementation of individualized nursing care plans to ensure that the needs of the residents are met.

Responsibilities:

  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the healthcare team to ensure seamless care and communication.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field; Less than one year experience; Graduate of accredited school of nursing (diploma).
  • License for Practical Nursing and AHA BLS CPR Certification.
  • Basic computer skills; Synquest; IV Pump; Phillips Monitoring System; Accucheck; EKG Machine, Specialty Bed Operation.
