About the Role:
CarePartners is seeking a highly skilled Registered Nurse Hospice Home Care to join our team. As a Registered Nurse Hospice Home Care, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.
Key Responsibilities:
- Provides leadership and facilitates the delivery of professional nursing care for the comfort and well-being of patients.
- Utilizes the nursing process of assessment, planning, implementation, and evaluation to provide, delegate, supervise, and document care and teaching of patients and family.
- Collaborates with interdisciplinary teams to develop and implement individualized care plans.
- Provides education and support to patients and families to promote health and wellness.
- Participates in quality improvement initiatives to ensure high-quality patient care.
Requirements:
- Diploma in Nursing, Associate's Degree in Nursing, or Bachelor's Degree in Nursing.
- Current licensure as a registered nurse with the North Carolina Board of Nursing.
- One year of acute care experience preferred.
