About the Role

CarePartners is seeking a skilled Registered Nurse to join our Hospice Home Care team. As a Registered Nurse, you will provide high-quality patient care and support to individuals in their homes, promoting comfort, dignity, and well-being.

Responsibilities

  • Provide professional nursing care to patients in their homes, assessing their needs and developing individualized care plans.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions.
  • Conduct thorough assessments, develop and implement care plans, and evaluate patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements

  • Diploma in Nursing, Associate's Degree in Nursing, or Bachelor's Degree in Nursing.
  • Current licensure as a registered nurse with the North Carolina Board of Nursing.
  • Compact license may apply, but licensee should confirm with NCBON.
  • BCLS certification.
  • Valid Driver's License.
  • National certification in area of specialty.

Preferred Qualifications

  • One year of acute care experience.
