Hospice RN Case Manager Job Description

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager at Haywood Hospice & Palliative Care in Fayetteville, NC.

About the Role

As a Hospice RN Case Manager, you will be responsible for delivering high-quality end-of-life care to patients and their families. You will work closely with the interdisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible care and support.

Key Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services
  • Identify patients' physical, psychosocial, and environmental needs and reassess as needed
  • Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team
  • Document patient progress and outcomes in a timely and accurate manner
  • Collaborate with the patient, family, and healthcare team to provide patient-centered care
Requirements
  • Current RN licensure in the state of practice
  • One year of clinical experience
  • Current CPR certification
  • Valid driver's license and access to a dependable vehicle
