Hospice Nurse Specialist

LHC Group is seeking a skilled Hospice Nurse Specialist to join our team. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care to individuals with life-limiting illnesses.
Key Responsibilities:
- Conduct initial nursing evaluations to determine eligibility for hospice services
- Develop and implement individualized plans of care to meet the unique needs of each patient
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Document patient progress and outcomes in a timely and accurate manner
Requirements:
- Current RN licensure in the state of practice
- One year of clinical experience in hospice or palliative care
- Current CPR certification
