This position is ideal for a skilled nurse eager to enhance their impact within the healthcare field, positively affecting the lives of individuals in their community.
Schedule/Hours: Flexible options available with 16 to 24 hours per week, including Day, Evening, or Weekend shifts.
Work Location: A combination of office and patient homes, with occasional flexibility for remote work.
Compensation: $45 per hour.
Key Responsibilities:
- Coordinate care for assigned clients, developing a goal-oriented care plan from admission to discharge, which includes ongoing assessments of client needs.
- Conduct supervisory visits to evaluate clients, families, environments, and clinical caregivers, ensuring comprehensive follow-up documentation.
- Oversee the availability and proper functioning of essential equipment and supplies for patient care.
- Deliver direct client care as necessary.
- Manage expectations and satisfaction levels among both internal and external stakeholders.
- Assess the quality and effectiveness of nursing practices and services, utilizing data to identify collaborative opportunities for enhancing patient outcomes.
- Provide nursing updates and secure re-authorization for continued care.
- Supervise, orient, train, educate, and evaluate clinical field staff continuously.
- Identify professional practice standards and areas for development within the organization.
- Contribute to the education and professional growth of staff, students, and colleagues.
- Participate in staffing decisions affecting nursing personnel, including hiring and termination.
- Ensure compliance with company policies, legal regulations, and professional standards.
- Maintain a professional demeanor consistent with registered nursing standards.
- Deliver nursing care adhering to best practices and professional standards.
- Base decisions on ethical principles, fostering a non-discriminatory environment sensitive to socio-cultural diversity.
- Participate in after-hours client care as needed.
- Promote a culture of quality and safety in client care through active participation in quality assurance initiatives.
- Assess clients and provide direct care as required.
Qualifications:
- Active RN license in the state of practice.
- Minimum of 2 years of hands-on nursing experience as an RN within the last 5 years.
- Ability to travel to patients' homes within the designated area.
- Reliable transportation and valid driver's license.
- Current CPR certification from AHA or ARC.
- Willingness to supervise nurses providing in-home skilled nursing care to various age groups.
Physical Requirements:
- Ability to communicate effectively in English.
- Must be able to travel; personal transportation is required.
- Occasional lifting of up to 25 pounds and ability to lift 50 pounds.
- Prolonged periods of walking, standing, bending, kneeling, reaching, and twisting.
- Must have visual and hearing acuity.
- Ability to respond physically and mentally to emergency situations.
Work Environment:
- Ability to function in diverse environments, potentially exposed to allergens and other conditions.
