Job Summary

We are seeking a highly skilled Pediatric Acute Care Manager LPN to join our team at Kintegra Health. The successful candidate will work collaboratively with Providers, clinical support staff, and other healthcare professionals to provide a medical home and optimize care for patients in a Team-Based Approach to Care.

The Pediatric Acute Care Manager (ACM) will be responsible for the daily prioritization of interventions on a list of required patient services on a provider's panel. The work of the Pediatric ACM will be driven by established quality benchmarks and clinical practices including, but not limited to, Uniform Data System (UDS), Patient Center Medical Home (PCMH), and Healthcare Effectiveness Data and Information Set (HEDIS).

The ideal candidate will have a minimum of three years of pediatric experience in a public health or an ambulatory care setting, and will possess excellent verbal communication skills, organizational, problem-solving, and critical thinking skills. Proficiency in Microsoft Office Suite and aptitude and understanding of a QI process methodology and proficiency in EMR documentation are also required.

The Pediatric Acute Care Manager LPN will be responsible for analyzing data for assigned panels, conducting pre-visit panel assessments, conducting provider follow-ups, conducting triage, and providing education utilizing the organization's evidence-based select criteria for education and triage. The successful candidate will also be responsible for maintaining knowledge of daily patient schedule, assessing availability and triage walk-ins, and coordinating quality improvement strategies with oversight from the Practice Manager and the assigned QI Champion as directed by the Kintegra Health QI Department.
