RN Clinical Manager, Home Health Operations Lead

About the Role:
CenterWell Primary Care is seeking a highly skilled RN Clinical Manager to lead our home health clinical operations. As a key member of our team, you will be responsible for overseeing the delivery of high-quality patient care, ensuring compliance with regulatory requirements, and driving operational excellence.
Key Responsibilities:
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct assessments and reassessments of patients, including updating care plans and interpreting patient needs.
- Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location.
- Review patient information, including disciplines required, to determine home health or hospice needs.
- Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing patient Plans of Care.
- Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers to ensure the appropriateness of care and outcome planning.
- Carry out supervisory responsibilities in accordance with Company policies and procedures.
- Participate in the interviewing, hiring, training, and development of direct care clinicians.
- Evaluate their performance relative to job goals and requirements.
- Coach staff and recommend in-service education programs, when needed.
- Ensure adherence to internal policies and standards.
Requirements:
- Graduate of an accredited School of Nursing.
- Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice, or equivalent environment.
- Home health experience, preferred.
- Homecare Homebase (HCHB) experience, preferred.
- CMS PDGM billing knowledge and/or experience, preferred.
