Job Summary:
As a Home Care Coordinator (LPN) at Kintegra Health, you will play a vital role in developing and implementing homecare services for our Senior Total Life Care participants. This position requires coordination of all Durable Medical Equipment and home care services in a home and community-based model of care.
Key Responsibilities:
- Assess the home care needs of a frail elderly population and develop specific plans of care.
- Coordinate 24-hour care delivery and implement all home care services to ensure quality services are provided to meet participant needs.
- Coordinate and authorize all Durable Medical Equipment (DME) and manage home supplies.
- Reconcile invoices for personal care service hours and home supplies.
- Perform acute, in-home visits as requested by primary care providers and/or supervisors.
- Support the Senior TLC mission to encourage and support the quality of life of seniors wishing to continue living in the community.
