Responsibilities:

  • Conduct home visits to assess patients' physical, emotional, and social well-being
  • Develop and implement care plans that address patients' medical, social, and educational needs
  • Collaborate with patients' healthcare teams to ensure seamless care coordination
  • Provide education and support to patients on topics such as disease management, medication adherence, and healthy lifestyle choices

Requirements:

  • Current NP licensure and national certification in family medicine, gerontology, or adult-gerontology primary care
  • Proven experience in home health care or a related field
  • Excellent communication and interpersonal skills
  • Ability to lift up to 30 pounds and navigate various dwelling conditions

The estimated salary for this position is $135,000 - $150,000 per year, depending on location and experience.
