Job Title: Residential Case Manager I

Job Summary:

The Residential Case Manager I at Bayview Hunters Point Foundation for Community Improvement is responsible for providing comprehensive case management services to residents, with a focus on helping individuals and families achieve stable housing and self-sufficiency.

This role involves assessing residents' needs, developing individualized service plans, and connecting them to community resources and services, helping them regain stability and move towards permanent housing.

Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency. This position reports to the Program Director, and works closely with the House Manager and other leaders.

Key Responsibilities:
  • Conduct initial assessments and intake interviews with new residents.
  • Develop and implement individualized service plans based on residents' needs and goals.
  • Provide ongoing case management, including regular check-ins and progress evaluations.
  • Maintain accurate and up-to-date case files and documentation.
  • Connect residents with appropriate community resources and services, such as housing, employment, healthcare, and education.
  • Advocate on behalf of residents to ensure they receive necessary services and support.
  • Coordinate with external agencies and service providers to facilitate access to resources.
  • Offer crisis intervention and support to residents experiencing immediate needs or emergencies.
  • Facilitate support groups, workshops, and life skills training for residents.
  • Assist residents with applications for benefits, housing, and other services.
Requirements:
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred).
  • Experience in case management, preferably in a homeless shelter or similar environment.
  • Can demonstrate a commitment to professional development and enhance candidacy.
  • Equivalent work experience acceptable in lieu of degree with a minimum of one (1) year direct experience in case management or related field.
  • Electronic data collection, input, and information maintenance.
  • Clinical supervision toward licensure is available for those on a BBS licensure track if selected.
  • Designing, planning, and implementing program services, with an emphasis on building accountability for program participants as they strive to move forward in their lives.
  • Ensure the health and safety of program participants' and thorough management of the physical plant; this includes safety, hygiene, appearance, and overall aesthetic of the facility.
  • Prepared to respond to emergencies with clear thinking and swift action.
  • Be committed to the mission of Bayview Hunters Point Foundation and possess a good working knowledge of and sensitivity to individuals with substance abuse and/or mental health issues such as PTSD, psychosis, borderline behaviors, bipolar, etc.
  • Certified Case Manager (CCM), Certified Alcohol and Drug Counselor (CADC), or Mental Health First Aid.
  • Bachelors degree in social services, psychology or a related field.
