Job Summary

St. Joseph Center is seeking a highly motivated and experienced Case Manager to join our team. The successful candidate will work closely with clients, community partners, and internal staff to provide high-quality case management services and support individuals in achieving stable housing and self-sufficiency.

Key Responsibilities
  • Conduct regular face-to-face case management sessions with clients to assess their needs and develop individualized service plans.
  • Collaborate with housing program managers and other case management staff to evaluate clients for permanent housing and assist with the preparation of forms and documentation required for housing voucher approval.
  • Conduct home visits and provide ongoing support to clients to monitor their progress and provide referrals to needed services.
  • Develop and implement individualized service plans that address clients' short and long-term housing needs, financial stability, educational, mental health, and other risk factors.
  • Ensure accurate and timely entry of case notes into the Homeless Management Information System (HMIS) and manage client file organization.
Requirements
  • Bachelor's degree in social work, human services, or a related field.
  • One to two years of experience providing services to highly vulnerable populations.
  • Strong knowledge of housing market, housing programs, and resources.
  • Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • Ability to communicate with and relate to a diverse group of people, including clients, community partners, and internal staff.
