Job Summary

The Case Manager will work as part of the Integrated Mobile Health Team - Full Service Partnership (IMHT-FSP) team, conducting assertive mobile outreach to chronically homeless individuals in the Venice community and surrounding areas. The successful candidate will provide intensive services to engage clients in the service continuum, link them to necessary resources, and assist with housing placement and retention.

Key Responsibilities
  • Develop close working relationships with outreach workers, case managers, police, paramedics, and collaborating agencies.
  • Locate and focus efforts on specific chronically homeless individuals who are most vulnerable.
  • Present findings to other collaborative agencies to develop a service plan of action that expedites housing opportunities and stability.
  • Assist with developing and implementing client treatment plans that emphasize coordinated strategies for addressing mental health/substance abuse issues, relapse prevention, symptom/behavior reduction, and medication management.
Essential Duties
  • Provide ongoing assessments of client progress in obtaining individual goals.
  • Utilize evidence-based practice models, including the Harm Reduction Model, CBT, and Motivational Interviewing, when working with clients.
  • Provide ongoing therapeutic case management services to ensure successful permanent housing, decrease social isolation, and prevent relapse risks.
  • Ensure a smooth transition into permanent housing by assisting clients with benefits and housing application processes, life skills, and accessing community resources.
  • Provide crisis management, case management, and other therapeutic services related to substance abuse and mental health services to clients on the streets, in service venues, in housing placements, or other locations as appropriate.
Requirements
  • Must be highly motivated and a self-starter, with excellent communication and organizational skills.
  • Strong knowledge of homeless services and resources, with demonstrated experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • Ability to work flexible hours, including some early mornings and evenings.
Qualifications
  • Bachelor's degree from an accredited college or university in social work, human services, or a related field.
  • Experience providing services to chronically homeless individuals or equivalent experience is desired.
  • Experience working with individuals with severe and persistent mental illness and/or co-occurring disorders is highly desirable.
