Job Summary:

As a Case Manager I, you will provide support and case management services to members served with mental illness and other co-occurring disorders, assisting them to live as independently as possible in their chosen community.

Responsibilities:
  • Provide direct care services to members served, including assessment, planning, and implementation of individualized service plans
  • Collaborate with members served, family members, and other service providers to ensure comprehensive support and care
  • Assist members served with daily living tasks, such as medication management, transportation, and meal preparation
  • Document interactions and progress in member's service plan
  • Participate in multidisciplinary team meetings to discuss member's progress and develop strategies for improvement
Requirements:
  • High School diploma or equivalent required; Bachelor's degree in a related field preferred
  • Experience working with individuals with mental illness and/or co-occurring disorders
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced, dynamic environment
  • Valid driver's license and reliable transportation
Preferred Qualifications:
  • Experience working in a mental health or social services setting
  • Knowledge of community resources and services
  • Ability to speak multiple languages
