Job Summary:As a Case Manager I, you will provide support and case management services to members served with mental illness and other co-occurring disorders, assisting them to live as independently as possible in their chosen community.
Responsibilities:- Provide direct care services to members served, including assessment, planning, and implementation of individualized service plans
- Collaborate with members served, family members, and other service providers to ensure comprehensive support and care
- Assist members served with daily living tasks, such as medication management, transportation, and meal preparation
- Document interactions and progress in member's service plan
- Participate in multidisciplinary team meetings to discuss member's progress and develop strategies for improvement
- High School diploma or equivalent required; Bachelor's degree in a related field preferred
- Experience working with individuals with mental illness and/or co-occurring disorders
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced, dynamic environment
- Valid driver's license and reliable transportation
- Experience working in a mental health or social services setting
- Knowledge of community resources and services
- Ability to speak multiple languages
