Role Summary:
The Clinical Supervisor plays a crucial role in overseeing patient care coordination and management. This position is responsible for ensuring that all services are delivered effectively while supervising clinical staff.

Key Responsibilities:
1. Evaluate case referrals and assess patient needs based on available information.
2. Assign qualified clinicians to each case based on specific requirements.
3. Conduct thorough reviews of patient cases, including clinical diagnoses and treatment plans.
4. Assist clinicians in establishing both immediate and long-term care goals.
5. Facilitate case conference meetings to enhance care coordination.
6. Perform quarterly record reviews and communicate findings to the Clinical Director.
7. Participate in the recruitment and orientation of new clinical staff.
8. Support the Clinical Director in developing educational programs for staff.
9. Contribute to the formulation and evaluation of organizational policies and procedures.

Qualifications:
- Current RN licensure in the state of practice.
- Graduate of an accredited nursing program; BSN preferred.
- Minimum of two years of management or supervisory experience in a healthcare setting, particularly in home care.
- At least three years of experience as a Registered Nurse.
- Strong communication and observational skills.
