- Provide Frequent Contact and Monitoring: Regularly check-in with consumers to monitor their progress, address concerns, and provide ongoing support.
- Accompany Consumers on Appointments: Escort consumers to clinical and supportive service appointments, ensuring they receive the necessary care and attention.
- Assist Case Managers: Collaborate with housing case managers and housing specialists during client interventions, providing support and guidance as needed.
- Participate in Case Conferences: Attend case conferences to discuss consumer progress, address challenges, and develop strategies for continued support.
- Maintain Client Files: Assist with the maintenance of client files, ensuring accurate and up-to-date documentation.
- Must be able to read and comprehend program materials at an 8th-grade reading level or higher.
- Must have adequate interpersonal skills to motivate and assist others.
- Must be able to communicate knowledgeably about local services and systems of care.
- Should be reflective of the population being served with one year of active participation in mental health self-help activities, and/or be a member of the HIV-positive community, and/or have active participation in substance abuse services.
- Peer services certification preferred.
- Valid driver's license, clean driving record, and access to a vehicle are required.

The salary range for this position is $18.00/hour - $22.00/hour, with salary offers commensurate with experience and other qualifications. The schedule is 17.5 hours/week, with in-person work based out of our Ronkonkoma office and field visits required.
