Position Overview

The Speech Language Pathologist I PRN is responsible for evaluating and assessing patients to set functional, measurable objectives aimed at enhancing their functional capabilities. This role involves delivering treatment in accordance with a structured care plan to achieve the defined objectives. The professional will also plan for patient discharge and communicate findings and progress in alignment with professional licensure, scope of practice, and departmental policies.

Key Responsibilities

  • Conduct thorough evaluations and assessments of patients utilizing both subjective and objective data.
  • Effectively communicate evaluation results to patients and their caregivers.
  • Implement treatment strategies as outlined in the established care plan.
  • Document all interventions and care provided in the patient's medical records.
  • Assess the effectiveness of the care plan and make necessary adjustments to the patient's functional status.
  • Provide discharge planning information, including education for patients and families, follow-up care, and referrals to community resources.
  • Contribute to the professional development of staff and students.
  • Engage in initiatives aimed at enhancing healthcare delivery.
  • Complete assigned objectives and responsibilities.

Qualifications and Experience

Experience: Entry-level position.

Licensure and Certification: BLS certification required within 14 days of hire.

Special Skills: Basic proficiency in computer skills, including keyboarding and navigating digital platforms.
