About the Role

We are seeking a skilled Medical Care Coordinator to join our team at Heartland Community Health Clinic. As a valued member of our healthcare team, you will provide comprehensive patient care, collaborating with our multidisciplinary team to achieve optimal health outcomes.

Job Responsibilities
  • Develop and implement care plans tailored to individual patients' needs, prioritizing effective communication and coordination with healthcare professionals.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with our medical staff to coordinate patient care, ensuring timely interventions and effective communication.
  • Maintain accurate patient records, adhering to regulatory standards and contributing to quality assurance reviews.
Requirements
  • Minimum 1-2 years' experience as a Nurse Practitioner or equivalent role.
  • Current State license as a Registered Nurse and licensed Nurse Practitioner, along with DEA licensure.
  • Demonstrated expertise in nursing practice and theory, with strong analytical skills and ability to assess patient conditions accurately.
  • Excellent interpersonal and communication skills, with a focus on patient education, emotional support, and collaboration with healthcare professionals.
