Role Overview:
The Registered Nurse (RN) provides professional nursing care to patients, coordinates day-to-day patient activities, teaches patients, and communicates with them via phone, in accordance with Heartland Community Health Clinic's mission, federal and state laws, performance objectives, and accreditation standards.

  • Manages acute, chronic, and episodic high-risk, high-volume patients of all ages to maintain or improve their current health state.
  • Oversees patient evaluation, treatment decisions, and consultations as required.
  • Develops and implements standards, policies, procedures, protocols, and guidelines.
  • Supports quality improvement efforts to increase patient outcomes and satisfaction.
  • Provides education to patients and family caregivers on healthcare resource use and documents it.
  • Coordinates services with other patient care locations and maintains communication with allied services.
  • Assists providers with technical procedures by preparing and informing patients.
  • Performs patient care procedures and administers medications as directed by the provider, independently.
  • Documents patient records, reports patient condition, medications given, and treatments administered.
  • Ensures informed consent has been obtained.
  • Communicates pertinent patient information to the healthcare team.
  • Reviews patient charts before and after being seen by the provider to ensure quality assurance.
  • Maintains equipment and supplies properly, practicing safety, environmental, and infection control methods.
  • Demonstrates compliance with clinic policies and regulatory requirements.
  • Takes phone calls from patients and family members, providing advice within scope of license and according to established protocols.
  • Schedules appointments and diagnostic testing with providers and specialty groups when necessary.
  • Interviews patients to gather information about their illness and determines priority of needs, records symptoms, reactions, treatments, and changes in patients' conditions.
  • Refills patients' medications per protocols. May transcribe physician orders for new prescription medication or changes to standing orders.
  • Assists resident physicians in becoming familiar with medical office procedures and surroundings.
  • Travels to various clinic locations as needed.
  • Supports clinic compliance with applicable federal, state, local, and HHS rules and regulations.
  • Participates in continuous quality improvement efforts to improve patient outcomes.
  • Works with clinical teams to develop and implement policies and procedures that maximize patient-centered communication and services.
  • Maintains patient confidentiality in accordance with HHS policies.
  • Reports building/equipment problems through the appropriate channels.
  • Performs assigned clerical duties and department-related tasks.
  • Attends staff meetings, department meetings, and other required meetings.
  • Maintains strict confidentiality.

Requirements:
Qualifications:

  • A current State license as a Registered Nurse is required.
  • Ability to lift up to 50 pounds.
  • Knowledge of growth and development principles and physical, emotional, and psychosocial needs of patients served.
  • Previous experience in an office setting or one-year experience in an acute care setting preferred.
  • Strong interpersonal skills to interact with patients, families, and physicians.
  • Basic Microsoft Word and Excel knowledge preferred but not required.
  • Electronic Medical Records (EMR) experience preferred but not required.

Salary Range:$25 - $40/hour
