Job Summary

We are seeking a skilled Registered Nurse to join our team as a Clinical Care Coordinator. In this role, you will be responsible for providing patient-centered care, coordinating day-to-day patient activities, and ensuring seamless communication with our clinical teams.

Key Responsibilities

  • Manage acute, chronic, and episodic patient cases, prioritizing their health needs and goals.
  • Oversee patient evaluation, treatment decisions, and consultations, ensuring evidence-based practices.
  • Foster collaboration among clinical teams, direct support staff, and indirect support staff to develop and implement policies and procedures that enhance patient outcomes.
  • Support quality improvement efforts, identifying areas for growth and implementing effective solutions.
  • Provide education to patients and family caregivers on proper use of healthcare resources, ensuring informed consent and confidentiality.
  • Coordinate services with other patient care locations, maintaining open communication with allied services.
  • Assist providers with technical procedures, preparing patients and documenting treatments administered.
  • Maintain accurate records and charts, reporting patient conditions, medications given, and treatments administered.
  • Evaluate patient charts prior to and after provider visits to ensure quality assurance.
  • Ensure safe and efficient use of equipment and supplies, adhering to safety protocols and infection control measures.
  • Demonstrate compliance with clinic policies, regulatory requirements, and industry standards.
  • Respond to patient inquiries and concerns, providing guidance within scope of license and established protocols.

Requirements

  • Current State licensure as a Registered Nurse is required.
  • Ability to lift up to 50 pounds.
  • Demonstrate knowledge and skills necessary to provide patient care suitable for the age group served.
  • Demonstrate understanding of growth and development principles, as well as physical, emotional, and psychosocial needs of the patient population.
  • Previous experience in an office setting or one year in an acute care setting preferred.
  • Strong interpersonal skills for interacting with patients, families, and physicians.
  • Basic Microsoft Word and Excel knowledge preferred but not required.
  • Electronic Medical Records (EMR) experience preferred but not required.

Compensation

$25 - $40/hour, commensurate with experience.
