The Coordinator will collaborate with various internal and external stakeholders to empower clients, enabling them to become engaged and productive members of society.

Key Responsibilities:

  • Engage in proactive outreach to clients requiring re-entry services.
  • Facilitate transportation for clients transitioning from correctional facilities.
  • Conduct initial and ongoing assessments to evaluate client needs.
  • Provide ongoing case management through regular meetings with clients.
  • Identify client strengths and challenges to formulate personalized improvement plans.
  • Assist clients in overcoming barriers related to their criminal history, including housing instability and employment challenges.
  • Connect clients with community resources such as housing support, job training, and mental health services.
  • Monitor client progress and ensure follow-up to assess improvements.
  • Track client milestones related to employment, training completion, and recidivism prevention.
  • Maintain accurate client records and ensure compliance with program requirements.
  • Collaborate with other programs to enhance service delivery for clients.
  • Perform additional tasks as assigned.

Qualifications:

  • Bachelor's degree in Criminal Justice, Human Services, Psychology, Sociology, or a related field.
  • Master's degree preferred or a minimum of two years of relevant experience.
  • Experience in case management, counseling, or related fields is advantageous.
  • Commitment to social justice and advocacy for individuals with criminal histories.
  • Strong communication skills, both written and verbal.
  • Able to work effectively with diverse populations, including those facing mental health challenges.
  • Ability to work independently while collaborating with various teams.
  • Fluency in Spanish is required.

Employment Type: Full-time

Compensation: $36,000 - $38,000 per year
