POSITION SUMMARY:

The Community Engagement Specialist delivers in-home and/or community-based assistance to individuals with developmental disabilities through SUBURBAN ADULT SERVICES INC. This role involves providing personalized support in one-on-one or small group settings, focusing on enhancing social skills, leisure activities, and daily living competencies to promote independence both at home and within the community.

REPORTING STRUCTURE:

This position reports directly to the designated manager or supervisor of the assigned service area and does not include supervisory responsibilities.

KEY RESPONSIBILITIES:

  1. Deliver supports and services according to the frequency specified in the individual's care plan.
  2. Ensure the safety of service recipients in both home and community environments, adhering to established safeguards.
  3. Coordinate service schedules with individuals and their families, in collaboration with program management.
  4. Effectively communicate with individuals, families, and community partners.
  5. Transport, accompany, and assist individuals in community settings.
  6. Identify unmet needs and provide suitable assistance to address those needs.
  7. Attend all mandatory meetings and training sessions.
  8. Complete required reporting, documentation, and service records accurately on a daily basis.
  9. Participate in all necessary training, including those mandated by managed care organizations and relevant health departments, demonstrating knowledge of service standards and regulations.
  10. Recognize and promptly report any safety issues, including those related to fire safety and the well-being of individuals and staff.
  11. Exemplify expected behavior in community settings, at work, and during recreational activities, maintaining professionalism at all times and adhering to the Code of Conduct.
  12. Ensure that all confidential and sensitive information is handled with the utmost discretion.

ADDITIONAL RESPONSIBILITIES:

  1. Comply with all agency policies and procedures.
  2. Exhibit sound ethical judgment and supportive decision-making as outlined in the Code of Ethics.
  3. Perform other duties as assigned.

QUALIFICATIONS:

To be considered for this position, candidates must be at least 18 years of age, possess a valid New York State Driver's License with an acceptable driving record, and demonstrate the ability to fulfill essential duties satisfactorily. The qualifications listed below represent the necessary knowledge, skills, and abilities required. Reasonable accommodations may be made for individuals with disabilities to perform essential functions.

NOTE: Candidates must meet background check requirements as specified by relevant authorities.

WORKING CONDITIONS:

This role requires travel to participant homes, meeting locations, and training sessions, which is a standard expectation of the position. Use of a personal vehicle may be necessary.

EDUCATION & EXPERIENCE:

A High School Diploma or GED is preferred. Experience working with individuals with developmental disabilities is also preferred.
