Job Summary

We are seeking a highly skilled and experienced Pediatric RN Case Manager to join our team at All Pointe Home Care. As a key member of our home health care team, you will be responsible for providing comprehensive nursing care to pediatric patients in their homes.

Key Responsibilities
  • Conduct comprehensive health and psychosocial evaluations of pediatric patients to identify their unique needs and develop individualized care plans.
  • Coordinate and provide skilled nursing care, including health promotion, disease prevention, and education to patients and their families.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
  • Supervise and evaluate the care provided by Home Health Aides and LPNs to ensure high-quality patient care.
  • Participate in case conferences and staff development meetings to stay up-to-date on best practices and share knowledge with colleagues.
  • Document patient care and progress in a timely and accurate manner, including submitting clinical notes, progress notes, and other required documentation.
Requirements
  • Current licensure as a Registered Nurse in the state of practice.
  • Minimum one year of experience in a clinical setting, preferably in pediatric home health care.
  • Knowledge of Medicare and Medicaid guidelines, as well as home health care principles and techniques.
  • Excellent communication and organizational skills, with the ability to work independently and as part of a team.
  • Valid driver's license and ability to drive to patient homes, which may not be wheelchair accessible.

Work Environment

As a Pediatric RN Case Manager at All Pointe Home Care, you will have the opportunity to work in a dynamic and patient-centered environment. You will be responsible for providing care to pediatric patients in their homes, which may require occasional travel to patient homes. You will work closely with other healthcare professionals to ensure seamless care coordination and communication.
