Pediatric RN Case Manager Home Care Specialist

We are seeking a highly skilled and experienced Pediatric RN Case Manager to join our team at All Pointe Home Care. As a key member of our home health care team, you will be responsible for providing comprehensive nursing care to pediatric patients in their homes.
Key Responsibilities- Conduct comprehensive health and psychosocial evaluations of pediatric patients to identify their unique needs and develop individualized care plans.
- Coordinate and provide skilled nursing care, including health promotion, disease prevention, and education to patients and their families.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
- Supervise and evaluate the care provided by Home Health Aides and LPNs to ensure high-quality patient care.
- Participate in case conferences and staff development meetings to stay up-to-date on best practices and share knowledge with colleagues.
- Document patient care and progress in a timely and accurate manner, including submitting clinical notes, progress notes, and other clinical record forms.
- Graduate from an accredited School of Nursing with a current RN license in the state.
- At least one year of experience in a clinical setting, preferably in pediatric home health care.
- Knowledge of Medicare and Medicaid guidelines, as well as home health care principles and techniques.
- Excellent communication and organizational skills, with the ability to work independently and as part of a team.
- Ability to travel to patients' homes, which may not be wheelchair accessible, and to perform physical assessments and provide care as needed.
