Role Summary
At Trinity Health, our RN Case Managers play a vital role in orchestrating patient care, evaluating needs, and formulating comprehensive care plans. They work in partnership with healthcare professionals, track patient progress, and facilitate seamless transitions across various stages of care while advocating for the patients' best interests.
Key Responsibilities:
- As a Registered Nurse RN Case Manager in the Emergency Department, you will lead a collaborative process that coordinates, oversees, and plans healthcare services throughout the patient's journey.
Essential Qualifications:
- Education: Bachelor of Science in Nursing (BSN) degree
- Licensure: Active Registered Nurse (RN) license in Connecticut
- Experience: Minimum of five years of diverse, progressive experience in acute care or related settings, with a solid understanding of nationally recognized medical utilization criteria, regulatory compliance, and accreditation standards.
- Certification: BLS certification is mandatory
