Job Summary:

The Child Welfare Case Manager will provide direct case management services to children, birth families, foster families, and primary caregivers. This includes assessing, planning, linking, coordinating, monitoring, evaluating, and advocating for the child and family's needs. The Case Manager will work collaboratively with a therapist, foster parents, primary caregivers, guardian ad litem, and other professionals to ensure the child receives the necessary services and support.

Responsibilities:
  • Develop and implement a case plan that meets the child's and family's needs
  • Conduct regular visits with children, parents, and caregivers to assess their well-being and safety
  • Provide support and guidance to foster families and primary caregivers
  • Collaborate with community agencies and mental health professionals to ensure the child receives comprehensive services
  • Attend court hearings and prepare and file court documents as required
  • Intervene in crisis situations and provide support to OHU/CBC staff
  • Meet with families in their homes, including evenings, nights, and weekends, in potentially high-risk neighborhoods
Requirements:
  • Bachelor's degree in a relevant field, such as Behavioral Sciences, Community Development, or Social Policy
  • State of Florida Child Welfare Certification (CWCM) preferred, but not required
  • Ability to obtain CWCM certification within 1 year of employment
  • Reliable transportation and willingness to drive daily for work-related purposes
  • Availability to include evenings, weekends, and on-call
  • Ability to be culturally diverse
