Our HouseCalls product provides annual clinical assessments to patients in the comfort of their homes, helping identify and reduce health risks while coordinating follow-up care to improve health and well-being.
Key Responsibilities:
- Conduct in-home assessments, including medical history review, system evaluation, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses for care management and active medical management in furtherance of treatment.
- Formulate a list of current and past medical conditions using clinical knowledge and judgment, incorporating assessment findings.
- Communicate assessment results to inform members' primary care physicians of potential care gaps.
- Recognize urgent and emergent situations and intervene appropriately.
- Educate members on disease processes, medications, and compliance.
Requirements:
- Active NP license or ability to obtain by start date, with unencumbered licensure and no open disciplinary actions.
- Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date.
- Ability to complete physical job requirements, including lifting a 30-pound bag and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
