At Optum, we're creating a new model of care that addresses the physical, mental, and social needs of our patients. As a member of our HouseCalls team, you'll play a critical role in providing annual clinical assessments to patients in the comfort of their own homes.



Key Responsibilities:
  • Conduct in-home assessments, including a review of past medical history, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screenings, including point-of-care testing as needed
  • Identify diagnoses and develop care plans to address patient needs
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination
  • Intervene in urgent and emergent situations, providing timely support to patients
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices


Requirements:
  • Active NP or PA licensure, with the ability to obtain national certification in the state of assignment
  • Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
  • Reliable transportation for home visit assessments


Preferred Qualifications:
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
  • Ability to travel throughout the state, with occasional opportunities for additional compensation
