Home Health Physical Therapist

LHC Group is seeking a highly skilled and dedicated Home Health Physical Therapist to join our team in Lufkin, Texas.
About the Role:- Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement treatment plans to improve and enhance patient well-being.
- Collaborate with the healthcare team to ensure comprehensive care.
- Evaluate patient functional status and physical therapy needs.
- Assist the physician in developing the therapy plan of care.
- Observe and report patient response to treatment and any changes in condition.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
- Instruct and inform the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
